I had the fork simply slip off at an intersection.
We looked at it and it seems like the fork needs to be about a half inch longer so that it cannot do it again.
New throw out bearing and clutch.
We put a little bit of wire to keep it on but that's just a bandaid.
We are thinking about welding tabs on the fork to make it longer.
Any input for me?

The shiny one used to look just like the dull one.
We made it longer and replaced or rebuilt every part from the fork to the pedal.
It was a halfassed mess.
